LPN Office Nurse / Float
Grace Cottage Family Health currently has an opening for a full-time LPN Office Nurse/Float. This position is responsible for the immediate care of patients; screening and assisting practitioners with exams and procedures. Excellent communication skills with patients, families and staff to ensure quality care while maintaining confidentiality a must. Current Vermont LPN license and a minimum of 3 years prior experience in an office setting required. Position is Monday-Friday.

The Pharmacist position will provide support to both the hospital and the retail side of Otis Health Care. The Care Coordinator is responsible for case management of patients
referred to the Community Health Team (CHT). This includes appointment
reminders, finding transportation and other agency resource referrals
and follow up; monitor appointment attendance; data collection and QI
monitoring; partner with providers, hospital and the community based
organizations to coordinate access to health care and social services.
Minimum of 2 years clinical RN experience. Experience coordinating
community services. Knowledge of existing community-based health care.
Successfully work with Blue Print providers, CHT, and other health care
professionals. Computer competency. Hold an active State of Vermont RN
License. Current healthcare provider CPR certification.
Part Time, 8-12 hours per week
Inpatient, Tuesday & Friday mornings Outpatient counseling, per patient load
Duties include but are not limited to: performing and documenting patient
nutritional assessments/reassessments; collaborating with medical, nursing and
other hospital staff to plan and implement patient care to meet the nutritional
needs of the patient; evaluating patient tolerance to therapeutic diet and
recommending modifications as necessary; approving patient menus; nutritional
oversight of the Dietary Department; and patient, family and caretaker
nutrition/dietary counseling in both the inpatient and outpatient settings.
BA/BS (or higher) degree in Dietetics. Current dietitian registration (by
national examination) through the Academy of Nutrition and Dietetics, with
Vermont certification. Minimum of two years experience in a hospital
Setting. Computer skills and familiarity with using an electronic medical
record essential. Excellent customer service skills. Strong team player.
